Itinerary: Rome -> Florence -> Venice -> Milan
I have always wanted to go to Italy, partly because of its history and culture, partly because of the Vatican, and this year that dream came true.
In early November, we decided to go to Italy on Thanksgiving. We only made a general plan:
Day 1: Travel to Rome
Day 2 - 5: Visit Rome
Day 6 - 10: Visit Florence area, including day trips to Siena and Pisa
Day 11 - 12: Visit Venice
Day 13 - 14: Visit Milan
Day 15: Coming home from Milan
Since it is off-season, I only booked two tours ahead of the time: the Scavi tour (Visit of the Excavations below St. Peter's Basilica) and the Vatican Museum tour. For everything else, we were just going to wing it, and it worked out most of the time. The only thing we'd like to see, but couldn't because we didn't have reservation was Da Vinci's Last Supper in Milan. So if you are going to Milan, and you absolutely have to see that painting, make sure you have reservation even in off-season.
