女兒9/16日(星期五)因腿部長了個瘡去醫療網內的醫生那兒看病,醫生局部小麻醉將患處放大,將膿血擠出一些,並取樣準備化驗, 然後開了抗生素,前後不到20分鍾。9/19(星期一)去複診,被告知星期五的化驗樣品丟了,得重新取樣(又痛一次不說,還耽誤了化驗的時間)。這次前後看病不知前後多少時間,估計不會長。周四,化驗結果出來後,說得換抗生素,所以又得去Walgreen拿新的抗生素,十天的大劑量。幾天前(十月中), 我收到了寄來的賬單,扣掉保險公司付的,自己要付$289多。我依照賬單上的電話號碼去查詢,對方說保險公司沒有全部cover,傷口清洗部分(drain)是自付額deductible部分。放下電話,就覺得不make sense, 每個月交那麽多錢(公司交的,加上自己每個payroll扣的), 這是手術費用,又不是什麽cosmetic related的東西,為什麽不cover, 為什麽是自付額部分?又拎起電話打回醫生處,問來了procedure code, 並告訴她,我準備跟保險公司交涉,給我點時間,不要把我的Bill送到collection公司。然後就準備打電話問保險公司。中間還有一些其他插曲,例如同事提醒我說, 因為Hipaa, 我不能打,要女兒親自打,或是要她簽一份release等等。 Anyway, 一番周折,跟保險公司詢問的結果是,保險公司搞錯了。這$289不應該我們付。
也因為這個賬單,我了解了這次看病的費用,概括如下:
看病(門診)包括清理:$562
化驗費:$166(一個細菌培養,一個什麽敏感性sensitivity化驗)
自己門診Copay:$25+$25=$50
自己藥費Copay:$25+$25=$50
加上保險公司cover的藥費(兩次抗生素,第一次不知是幾天的量,第二次是十天的量,加起來一定不少於兩百塊),這樣一次小毛病費用要$1000刀,雖然自己隻付了$100多點,但我們可以從中知道個大概。
醫療保險曆來都是個大話題,總統競選少不了它,改革啊,全民醫保啊,拉人心,拉選票。對公司而言,這也是個很大的開銷,每個月員工的保險費是很大一部分支出,如果公司財務狀況不好,除了裁人,接下來就是通過改變保險計劃,如提高員工的自付額(deductible), 而悄無聲息地降低公司在醫療保險這部分的支出。從家庭角度,那就更是與自身利益緊緊相關的。一般來說,夫妻最好有一個在公司做,那種benefits比較好的公司,這樣就能享受到好的保險,否則個人自己買,貴不說(除非你eligible for obamacare),很多時候還進不了好的醫療網。我有個朋友,她得自己買,一個月付$400多塊,據說今年又給她漲了一百多塊,20%多的漲幅,這樣的保險,還幹不了事。她頸椎壓迫神經,造成手腳麻,醫生建議做MRI,她說做一個$2000多,全部自付,保險不cover,遲遲不肯做,說要等回國內再做。唉,這樣的保險有什麽用?
再說,保險公司其實是挺黑,常聽人說,你做個檢查,跟做的地方negotiate,說,我不想通過保險公司做,能不能便宜,結果還居然會更便宜。保險公司是不做賠本買賣的,他們有underwriting部門,每年重新估算,如果今年的理賠成本(claim cost)超過他們的premium收入,那明年就會調高premium rate,所以,羊毛出在羊身上。另外,保險公司裏的中介broker,拉一個客戶(如一個公司), 介紹成一筆,月月拿傭金(不是一次性的喔),穩坐穩吃,你想這錢哪裏出?羊身上出啊。

文章 說60%的美國人擔心他們退休後的醫療費用。這其實是不足為奇,美國的醫療昂貴是早有所聞的,貴的可以讓你一夜間傾家蕩產。現在人的壽命越來越長,需要long-term care 的人越來越多,美國的醫療前景不容樂觀。所以,從這個意義上說,健康地活著就是在賺錢。不是有句說法,年輕時拚命賺錢,老了又拿錢去買命。這一定不是我們想要的。我們不能改變這個社會、這個製度,我們能做的,就是最大限度地讓自己健康地活著,從現在開始,愛惜自己的身體,遠離疾病。
Where shall we retire?
An article in thestreet.com lists " 25 best cities for Retirement Health care" in the U.S. Among them, nine of them are in CA, with Salinas topping at No.2. Although the ranking, provided by the U.S. Department of Health and Human Services (HHS) and JCAHO, is based on the the health care needs and facilities, it raises a question to me. Is CA really such a good place to retire? Where would I retire in decades?
CA is well-known for its sunshine, beaches, wineries, cultural vitality, food, etc. It has warm weather year-round, ideal for retirees. But the high living cost is the main drawback. Look at the property tax bill we recently got. Assuming we are debt-free by the time of retirement, we still have to pay this kind of tax. And if the property value keeps inflated, so will the bill. Think of the mostly million- dollar houses in northern CA, their property tax alone can scare us, not to say there are other expenses, unexpected medical expenses for instance. People may suggest selling the house, but then what about the life quality?
I actually never thought about my retirement. It is still too far away. Maybe I will end up just staying here. Or maybe I can go back to China, or maybe....But in reality, there is no perfect place to retire, or "there is not one place in the world that everyone is going to love." While there is nothing else we can do now, we can always choose to keep fit and stay healthy wherever we live.
