很奇怪沒做neck dissection.

She is borderline of high risk, whether she is indicated for RAI or not, please see the following criteria.  Most people follow these recommendations.

RAI is recommended if any present:
Extrathyroidal extension
Tumor >4 cm
Suspecious for metastatic disease

Consider RAI, if any present:
Tumor 1-4 cm
High risk histology
LVSI (+)
Multi focal
Minimal extrathyroidal extension
N(+)
Inappropriate postop Tg
