MRI Report:
1.各腰椎骨質增生；各腰椎間盤變性；腰1/2，2/3椎間盤膨出；腰3/4，4/5椎間盤膨出並向後突出。腰5椎體上緣見schmorl結節形成，相應節段硬膜囊及神經根受壓。all lumbar vertebra hyperosteogeny +, degerative changes in all lumbar intervertebral discs +; L3-L4, L4-L5 posterior disc protrution. schmorl nodes on L5 anterior limbus vertebra with compression on dural sac and nerve root
2. 骶管囊腫: sacral canal cyst
EMG Report:
1.雙腓神經，左脛神經，右股神經傳導速度正常；右脛神經感覺纖維輕度軸索損害，運動纖維傳導速度正常。
normal conduction velocity in both peroneal nerve, the left tibial nerve, and the right femoral nerve
mild damage in Right peroneal sensory fibres, but normal conduction velocity in motor fibres
2.左股二頭肌電圖未見異常。右股直肌，股二頭肌，脛前肌肌電圖神經性損害（L2-S2根性損害較大）
no abnormalities in electromyogram of the Left biceps femoris muscle.
mild nerve injury in Right rectus femoris, biceps femoris and tibialis anterior muscle, (especially L2-S2 root injury)
