with your manager:
#1. go through the items in the review form, reach an agreement and sign it off or do not sign it off with additional revision/modification.
#2. Assuming that in the reviewing form, for each item, your performance is taken as "exceeding expectation", then you have a very good chance to ask "what is your plan for this group for next year? What you think that I may provide more help with...I have some thought about it and would like to have your valuable advice: blah, blah....
#3, Normally, aroud this time, co-op goal is also released, try to align your personal career pursuing with it; present your plan to make him/her believe that your promotion will benefit your co-op (or his/her plan for your group) to achieve the goal. For example, if she/he can focus on other side of her/his 兼職-ed (?) responsibility.
The best way to resolve it should be through your manager. In any event, if you have a chance to talk with your manager's manager, the perspective is the same. (the #1 can be removed here; your review will not be signed by the manager's manager, right?)
