Is there a self assessment section in the performance review section? Or a section for you to compare your goals vs. achievements? That's where you put what you have accomplished over past year. Your manager will read this section first before he/she completes your rating. Not a common practice to send job description to the managers (I guess you meant your roles and responsibilities over past year?), especially those you have not touched base for a while. Next time, have regular 1:1 or lunch with those people to keep the networking, but be sensitive to your direct boss's reaction to this.
