胸水

 

在去年底的PET-CT掃描中即報告有右側少量胸水，當時醫生認為可能與浮腫相關，是化療藥的副作用之一。（參見“浮腫”篇）

盡管上半年種種努力把小腿浮腫搞消了，但胸水一直都在，近二個月更甚，已經到了影響呼吸的程度。

還影響到循環係統。在由立或坐位躺下的時候，心跳加速，腦子有充盈感，麵部充血，呼吸困難。大喘幾口氣可以緩解。起立時倒沒覺得有問題。

2023-8-18日 CT報告提到

right moderate and left loculated moderate pleural effusion with consolidation and or atelectasis of the lung bases.

而且腹腔內亦有變化，醫生考慮是否有轉移、或第二原發癌。

後天將抽右側胸水，看看是否能改善呼吸、循環。同時檢查脫落細胞和生化分析。

 

8-31日，麻醉下抽右側胸水，a total of 1500 mL of yellow fluid was removed.

部分胸水送病理及化驗。

細胞學檢查：Rare atypical cells present. Insufficient for additional testing

Protein, Fluid（mg/dL）Value 4,100

LD, Fluid （IU/L）Value 145

Glucose, Fluid （mg/dL）Value 117

 

細胞及分類、培養：少量白細胞和紅細胞。

Agree with differential cell count. No definitive microorganisms or malignant cells are seen in the material examined. Correlation with microbiology and cytology results is suggested.

 

抽水後呼吸輕鬆很多，咳嗽也輕了。不是血性胸水，沒查到癌細胞，比預想的要好。

腫瘤醫生對上述結果不置可否。

 

10-25日 再次看醫生，提到呼吸急促和咳嗽，要求做胸透（chest x ray)：

報告提到：Small right pleural effusion. Compressive atelectasis of both lungs。

 

10-27日 時隔八周，再次抽右側胸水，a total of 1000 mL of yellow fluid was removed.

Medical Cytology Interpretation

NEGATIVE FOR MALIGNANT CELLS

Mesothelial cells and few lymphocytes.

 

沒有找到惡性細胞，是個好消息。不過胸水源源不斷，說明右胸還是有問題。
