I've never heard a native speaker mixed up "he" and "she". Simple, they are very sensitive to this and tense. Mixing up these words would completely mess up the intended message and give the listeners wrong ideas. Probably more and more people would say "less people", but they'll never get confused by "he" and "she", not even in their sleep.
