知道您們在文學城德高望重。現急需您的幫助。我自己辦了PERM，可對自己辦綠卡沒有信心。雖有pijiang老師的文章做參考，但仍有不少疑問。敬請幫助。
1).I－１４０ 表：part３，14b Passport number 是指新護照還是當時入境時的舊護照？２００３年最後一次入境時用的護照早就更新了。
-It is current valid passort.
Part ５，３a，If an individual, give following: 此處的 individual 是指誰？
-Leave it bland because it means the petitioner of self petition (e.g. NIW/EB1A).
2). I－４８５ Part１， 最後一格，此處的 expires on (mm/dd/yyyy)是指 I-94 status expiration day 還是指 lawful status day?
-It is the legal status expiration date (normally it is I-94 expiration date). If your status is F-1, then it is the I-20 expiration date.
Part ２，Application Type:
是填 h 並寫上 Form I-140 being filed concurrently with this form 嗎?
-Choose "a" even if you don't have an approved I-140 yet.
您的文章說 h於以屏棄， 我不明白是何意思。
3).I－７６５,１１，我曾經申請過OPT，但我弄丟了那個卡。您在文章中說可以用護照代替。是用當時的護照copy嗎？
-It is your current valid passport.
4).G－３２５A， 是一共遞交４ 份嗎?
-only need one copy (for I-485).
5).I－１３１屬於哪種 Application type？是屬於I－４８５pending 嗎?
-Yes.
您當時的文章說不付費，我查了網站，收費 $360,對嗎？
-No fee for I-485 based EAD or AP.
6). PERM copy， 是否僅遞交首頁（cover page)和 officer 簽字的二頁即可?
-Provide the copy of all pages.
7). 除了I－１４０ 和 I－９０７，最好用公司的支票， 其它是否用我自己的個人支票即可？
-Yes. By the way, you can not use premium processing when you file I-140 and I-485 concurrently. You may file I-140 using premium processing, ten file I-485 after the I-140 is approved (it takes only 15 days, anyway). Also, if you were born in Mainland China, you can not file I-485 until the priority date is current!!

8).我想辦 I-140加急 , 請問是把所有的表格及有關文件都送到USCIS Texas Service Center (4141 North St. Augustine Road, Dallas, TX 75227) 嗎？
9).我沒有出生證明，請問怎樣做 birth certificate substitutes? 我暫時不交 birth certificate substitutes 可以嗎?
-You need to provideit with I-485 filing, not need it for I-140.
10).最後一個最重要的問題, 我老板需要交前三年的稅單嗎？如稅單顯示（I-140 form Part 5, 也要求填年收入）公司無經濟能力支付我的工資，有無辦法補救？
-Your company need to provide the evidence to prove the ability to pay, such as tax return, financial statement, etc. Otherwise, the I-140 may not be approved.
