2. 二流精英醫院：
當地醫學院直屬的教學醫院。
注：大多醫學院也都與一些社區醫院有附屬或合作關係，便於醫學生了解社區疾病譜及處理等等。
3. 一些州府級城市，雖然不是”大“城市，但也有一些三甲級的社區醫院。它們的醫療綜合能力並不差。很多這類醫院實際上也是住院醫生培訓點，屬於教學醫院。
4. 廣大社區醫院，比如county hospital，其實有些county hospital 並不小，可以劃在第三類別裏，比如美劇《ER》，就是以芝加哥的Cook county hospital 為原型的。
