You sure there is no stones that are colonized with bactereia right? If you failed outpatient oral antibiotics, get admitted and get IV antibiotics

"驗尿驗血， 都有感染", how so? WBC in blood 10.9 is within normal limits. What is the WBC count in urine sample? >50, call it UTI, otherwise not impressed.

The discomfort might come from the scarring. See a urologist to rule out stricture.
