乳腺癌求助--- update: 關於化療方案的糾結

從確診，手術到現在馬上要開始化療，在這裏得到很多專業的建議和令人溫暖的鼓勵，非常感激。我這還真是一個令醫生都感歎的“complicated case” ，經常有新的信息出現，心情也跟著跌宕起伏。

 

我是全切，前哨淋巴隻切了一個，有isolated tumor cell, 但醫生說按現在的標準算Negative。

 

biospy時發現的兩個tumor：

乳頭處的tumor HER2是negative (IHC score 2+，FISH negative) ；

12點的tumor HER2是positive (IHC score 3+)。

ER,PR都positive.

 

手術病理顯示有兩塊 specimen, 因為有re-excision。

但是隻發現一個 tumor(乳頭處)，1.5cm, HER2結果和biopsy時乳頭處的tumor一樣， IHC score 2+, FISH negative，另一個(12點)不僅tumor沒找到，biopsy時放到那的clip也找不到。後來作過X-RAY， US， MRI，沒有發現體內有什麽殘留。

 

本來已經打算開始chemo， TCH*6 with H for 1 year based on the fact that ONCOTYPE SCORE for the tumor at nipple is 20 (intermediate) and there is another tumor which is HER2 positive, 結果今天醫生來電話說second-opinion pathology review發現biopsy時12點的tumor HER2居然是negative (IHC score 2+， FISH negative) , 所以他要取消Herceptin, 讓我隻做TC*4，  我一下又暈了，這HER2 test也太波動了，不同實驗室作出的結果相反，可能嗎？

萬一其實是positive, 我卻沒用heceptin, 不是很慘嗎？

另外即使算HER2 negative, 我應該用ACT還是TC呢？

是不是應該再找第三方作一次her2 test, 沒有醫生order,  我也沒法找人作吧？

況且各種test拖來拖去，我已經是手術後7周了，真想馬上開始chemo 。
