醫生或醫院向保險公司申報時，一般高於正常費用的2-3倍。

如果申報時低於正常收費，保險公司會付得更少。

Then the insurance compay will pay what it should pay.

For physican claims $300 for first visit (CPT 99222), the insurance will pay $90. If you have copay, you have to pay up to 20% of copay. The insurance 's EOB will state it. if you have paid all of your copay, no thing to do.

CHECK INSURANCE COMLPANY'S EOB. 

It will state how much is your copay. If say no copay, you do not need to pay anything.
