Thanks for your guys kind suggestion.
on the phone interview the hiring manager already asked me why I left the previous emlpoyer. I didn't tell him I was kicked out. He was fine with my answer.
Now it is just the forms. I am gonna leave it blank unless they insisted it.
Now I have to think about how to give reference if they ask for it.
