1. phone. face to face would be better. email gives one shot only;
2. hiring manager, if you could. most people won't be able to;
3. I'd ask the gap. this allows you to provide more info (and that's also why you want to do phone interview)

in all, don't give up. you need to keep the connection even if you do not get the job this time.
