小女子又遇到新問題，please help me!!!!!!

This is my last week to work.I am waiting for laid off, and I have a doctor appointment tomorrow.If I see docotor tomorrow, the doctor will give me sick leave for at least one week, then I will take sick leave tomorrow, when I will sign terminate document? I work in state university. Very appreciate your answer!!!!!!!!
