Talk to the person who you directly report to.

In most cases, it doesn't matter how many people, who are at the same level as your boss, were involved. You should always ask advice from you boss, that is, the person you directly report to. S/He is also responsible for the errors in the report. S/He should know how to handle it. If s/he doesn't, it means that s/he is not qualified for the position. You shouldn't worry too much.

Don't cc to other people. It is inappropriate.

Don't be afraid of making errors or mistakes. Everybody makes mistake. It is the time to show that you have the ability and the skill to handle mistakes. People who can't handle mistakes will have no chance to go higher.
