Can you go to yoru former manager?

If it is possible, maybe you can do an internal transfer.
but looks like you have to leave your current position. You can disagree with the performance review. But I would not file a complaint about your manager. You do not want to make it on record with HR that you are a trouble maker. Better leave with grace and keep the door open for future.
