Please advise: on salary history

I am new in the US--I would rather to type in Chinese, but for some unkown reason, I couldnot switch to Chinese. I am currently apply for a job in NYC, and it states that I have to submit salary history, along with cover letter and CV. i had little experience looking for jobs, and i wonder how you all handle this. Do you have to very honest and concise on the amount of money you made from previous job positions? Or you can just give a rough range which may reflect your own salary requirement for this specific job you are applying to? Thanks for reading.
