It could be a loose or poorly fitting cover to the air filter, which often follows a quick oil/lube service. Or the vacuum hose that fits to the vent solenoid is loose or off, often from the same type of service. The solenoid and vacuum ports are often attached to the air filter cannister.

I have fixed this code twice on our 2001 Highlander, and the total cost was $0. The two conditions were the same as described above.

After locating and fixing the problem, the codes are reset this way (at least on the 2001 Highlander):
1) Open fuse box under hood
2) Locate EFI fuse using the diagram on the box
3) Turn starter switch to ON without cranking the engine
4) Pull the EFI fuse (fuse puller often attached to the box)
5) Wait at least 10 seconds, then turn the starter switch to OFF
6) Reinsert the fuse
7) Start the car

Check Engine light will be off if the problem was fixed.
