insurance company. They somehow got your information (it is illegal to give patient info to third party, I wonder any health insurance company dares to do that), and they want to make money from you, that's all.
Insurance company won't bother for a member who costs 10K - 20K a year, big cost to you, but piece of cake for insurance company. The rules of thumb for health insurance company: 10% member contributes to 90% of the cost. There are people cost millions a year.
You shouldn't worry about anything. Tell the nurse group: do not call again.
