Normally, insurance company and doctor has a "set price" for a specific service. In other words, the doctor can only charge the patient/insurance company for certain amount of money. Your insurance company should pay all the charge less co-pay (fixed amount or percentage, depending on your policy).
What did your insurance company say about this bill? Have you received an explanation of this charge from insurance company? Did your doctor give insurance the "correct code" of the service?
