Usually, by contract, hospital has to claims to insurance within 365 days, after that, insurance company is not responsible for the claims.
The hospital's claims department didn't the claim on time to your insurance company. It's their own fault. You are not responsible for the claim.
But I don't know how to solve this issue legally.
