Just want to correct you. But I am not sure if we are talking about the same thing.
Before you actually get the service, you may ask your doctor to do a "pretreatment estimate". Just like he has already done it and send the right code to your insurance company. Then your insurance company sends back to you or the doctor and tells you how much they will cover.
We had done that for several times with HMO or PPO.
