In your description, insert as much facts as possible, such as vehicle license number, addresses for x and y, police report date/serial number. etc.  Use paragraph to separate your charges, such as: 

Driver x, negligence in driving an automobile.

Owner y, negligence entrustment of uninsured automobile to driver x.

Seek damages from x and y jointly and severally.
