there should be certain verifications before finalizing the transaction. Usually in your case, you should get an email (if not real time verification) or warning (real time verification) with "failed to finish the trsanction because of ..." If the name, address ... you input has nothting to do with the transaction, why do you need to input?
If you think A bank didn't have anything wrong, then you should just accept the fact. It's your money, your call.
that's why I said it's "system error", a BUG in the system.
