This was an online wiring, I finished the transaction by myself. If you transfer some money out of your account to another account online, having account# is enough, the online banking system doesn't check the name at all.
The correct and wrong accounts are all in bank B so bank A has no way to know if the name/address match the account#. However bank B should take some responsibility I think.
