I have almost the same experience as yours regarding Cigna

Cigna refused to pay a lab test bill because they said the lab didn't get a correct authorization code from Cigna. And Cigna also told me to ignore any letter from that lab test provider. But this doesn't make any sense as the provider keeps sending me bill and phone calls. I'm still negotiating with cigna and the provider. but I understand your feeling, this is very annoying and frustrating.
