Thanks a lot for your reply.

1. I didn't find the definition of family member in
the instructions. Could you please tell me if this
include parents and siblings? And if a family member
is deceased, do I still put it in?
2. For second one, for the years of school
successfully completed? does this include the years
only with a degree or both ones with a degree and the years towards a degree?

I really appreciate your help.

2. The full name of this choice should be "univeristy/college", of course graduate education should be included.
