question about EAD card renew

All teachers: I am the primary, Now I need to renew my EAD card.My wife started working with OPT at 1/2011, and stopped working at 9/2012,now she stays at home.

1. In case she needs to work later, do I need renew her EAD card too? or she can get new card    

   with mine automatically? 

2. If I need, #15 in I-765 form, do I need enter unemployee?

3. and which code do I need enter for #16 question?
