I checked NYC general consular, and found info below:
應提交的其他材料:
１、聲明書文本；
２、離婚的，應提交離婚證或離婚調解書或離婚判決書原件及複印件；喪偶的，請提供原配偶死亡證明原件及複印件。
具體要求:
1、當事人應有完全民事行為能力；
2、當事人親自申請並在領事官員麵前在聲明書上簽字；
3、婚姻狀況聲明書公證的有效期一般為六個月；
4、未婚人士如處理房產、辦理戶口等事項需申辦無配偶聲明書公證，應在聲明書中說明此證明僅限用於在國內辦理相關事宜；
５、有特殊情形的，應根據使領館要求辦理。

Not having seen your mentioned 單身證明書, I believe when you have such document not notarized, you could still require "或" - to be notarized by "由中國駐該國使領館出具公證".
When you fill the 單身證明書, only sign/date it in front of a notary, it will be stamped (refer to 公證書). If no space available on above document, print it on leagl size paper to make more room. This is "可". In case this document needs to be sign in front of consular officer, just sign it again.
中華人民共和國駐外使領館公證認證申請表 - It looks to me to bring this completed document and sign in front of consular officer. It does not appear it requires notarization.
