H1-B和H4的延期

請教高手，

我的H1-B今年10月到期（6年），I140 已批，想申請另外3年的延期，如果現在動手延期（premium processing)，新批準的到期日是2014年4月還是2014年10月？

老婆是H4，計劃7月回國，然後再回來，需要重新去美國使館簽證。我延H1-B的時候是不是要把她的H4也一起延了以獲得一個新的H4 I94？對她在美國使館簽證有幫助嗎？還是她可以直接用我已經批準的H1-B（2014年4月或10月）直接去使館申請H4？
