Your personal qualifications need to be on the official form and signed. Think of Schedule A nurse, or even NIW, they can skip the LC/PERM with DOL, but still need to provide filled LC/PERM form with I-140 to USCIS. (NIW only need personal part, schedule A and sub labor please fill both parts if possible)
