Go through LC based, because NIW physicians is not the same as NIW for researchers. NIW physician must have 5 years working in designated areas(medically underserved) before I-485 can be approved based on this category. However J1 physicians can get the Conrad IGA waiver and the three years can be counted into the five years of NIW physicians. Or if you choose to do NIW as researchers, then NIW and get GC first and then do residency. Do residency while pending I-485 based on regular NIW may hurt your I-485 case.
