ask PJIANG ,XBT about I-765 填表問題(情況複雜),謝了

I am a H1B holder. 我去年7月隨employment based 485同時申請的EAD/AP（來自CSC Service Center) 今年9月過期。I have never used the EAD until now.
今年二月份提交了婚姻綠卡申請（I-130+I-485)到Chicago USCIS lockbox ，老公是公民,因考慮到去年申請的EAD/AP 今年9月份才過期，所以沒有隨 婚姻綠卡申請同時遞交EAD/AP 申請。現已收到來自MSC(national benefit center)的I-130和I-485receipt.準備在舊的EAD/AP expire 前申請新的EAD/AP（基於婚姻綠卡申請, family based 485）。現填I-765表有以下疑問：

First question: 因以前的EAD卡是基於employment based 485(submitted before July 30,2007)，　這次申請EAD想提交基於婚姻綠卡申請的I-485($1010) receipt copy 從而省$340 元的I-765 application fee,但不知這次申請是屬於基於婚姻綠卡申請的initial EAD application 呢？還是在上次employment based 485上的renewal?

I-765 表的表頭：選１還是３？
I am applying for:
1.Permission to accept employment
2.Replacement (of lost EAD)
3.Renewal of my permission to accept employment(attach previous EAD)


Question 11: Have you ever before applied for employment authorization from USCIS? I will choose “Yes”

Which USCIS Office?
I initially applied and submitted my EAD application to NSC last July, but received EAD from CSC service center last September. So which USCIS office I should put here? NSC or CSC?

Question 16: If 基於婚姻綠卡申請( family based 485) 的EAD 和employment based 485 EAD 申請，the Eligibility Categories 都選　(c) (9)?
