I am applying for: 1.Permission to accept employment 2.Replacement (of lost EAD) 3.Renewal of my permission to accept employment(attach previous EAD) - Renewal Question 11: Have you ever before applied for employment authorization from USCIS? I will choose “Yes” Which USCIS Office? - NSC I initially applied and ted my EAD application to NSC last July, but received EAD from CSC service center last September. So which USCIS office I should put here? NSC or CSC? Question 16: If 基於婚姻綠卡申請( family based 485) 的EAD 和employment based 485 EAD 申請，the Eligibility Categories 都選　(c) (9)? - Y
