It happens quite several times recently as I am aware of that CBP officers made such mistakes.
You don't need to extend his H-4.
Using the link in previous post, contact the CBP office at the port where your kid entered. They will provide you a phone number (not published) of the CBP located in the terminal which associated with the airline bringing your kid here.
Call that number to request a supervisor. The supervisor knows how to handle it.
You need to be presented at the terminal physically, with his passport, I-94 and her I-797. Call supervisor when you are at the terminal so you can be allowed inside. Explaining the situation, your kid will be issued a new I-94. Make sure that the supervisor has also updated the info in the computer.
Make a copy of I-94 before you go to CBP at the terminal.
