請教！What do you have to do for the TB test REF?

I had medical Exam on Feb. 2007. The Doctor thought all of Chinese will get positive, so asked us to do X-ray directly. The questions are:(1) Do you get TB test ref due to no skin test? What did you do for the ref? (2) Do you have an attorney to handle your case? If yes, does that mean attornay only takes care of I-140 and I485 filing? Will you get all of letters and ref from USCIS after your I-140 was approved? Thanks!
My 140 was approved last month, I don't know whether my attorney will continue to take care of my case in case I need their help.
