we had x-ray and still asked us to do the tuberculin skin tests. From doctor's point of view, it is ridiculous because it is not necessary at all. But the people working in USCIS know nothing about it.

We only hired the attorney for I140. You can file I485 by yourself. Just follow the instructions, it is not hard.
