A. All application fees (three checks in the amount of $325 for I-485, $70 for fingerprint)
B. Form I-485 with two identical, natural color passport-style photographs
C. Form G-325A.
D. Sealed Form I-693 of report of medical examination
E. Copies of passport with visa page, I-94 card (front and back) and all notices of approval for H1-B
F. Notarized birth certificate
G. Notarized marriage certificate (for my spouse)
H. Bank statement letter, employment letter, copy of pay stubs for most recent three months and W-2 Form (not necessary for NIW or EB1a)
