which is ist step for waiver?

我查了DEPT. OF STATE and 紐約總領館有關WAIVER 的PROCEDURE, 不盡明了. Which is first step,向美國國務院申請豁免編號（Case Number）or 領取《留學人員豁免申請表》? How to apply case number? Need I fill out DS-3035 for this case number?

Moreover, Does this application include J-2 ? Yes or no?
