my LC was approved from DBOC and I have couple questions

My LC was approved today from Dallas backlog, finally, (my PD was 03/18/2005) first, my first question is how can I find out which category I belongs from their paper? second, I do understand that I have get though the medical check, but I do not have any immunization record, where should I take those shots, in addition, I saw someone post on the board that some immunization shots takes months in order to get result, is that true? I hope not, I want to get everything process on both 140 and 485 while my PD is current according DOS' visa bulletin. anyone can answer my questions?
