medical check should be easy...check the USCIS website,they have docoter's list.
in Dallas, most people go to a chinese doctor Jay Meng.
just 2 days or at most one week to get it.
also uscis has processing time link for each center. but now you can only file your 140/485 to NSC, and they will transfer your file to other center.
