Date of arrival should be 01/03/2006. not the first entry date.

I-94 number: Last Action Rule plays here - compare with your entry date on I-94 card and your H transfer petition approval date on the approval notice, you should enter the number on the document which the latest action taken. Say: your I-94 was dated 1/3/06, your H approved on 1/4/06, you should use approval notice I-94 number; if I-94 dated 1/3/06, your H approved on 1/2/06 even though you received after 1/3/06, you should use I-94 card number.

I-94 number can be different if you travel subsquently and each time of entry you obtained a new I-94 card. When you extend your H, you should alwasy put the most recent I-94 number. Or you can choose to put the same number you have obtained before onto I-94 card when you subsquently enter the US by cross the number on I-94 card and ernter the same number. This way you always keep a same number.
