We had same experience for my parents-in-law. They were requested to provide the extenion notice last year when they apply for visa in Beijing. We used the I-824 and waited for about 6 months, INS sent a reply told us the extension approval notice had already expired(2001 too), so our request for duplicates were 'rejected'. How rediculous!
I still can't figure out a way of getting a duplicate for the lost extension notice. In my case, I may just attach the 'reject' notice for duplicate if my parents-in-law need to go for visa inerview some day.
