I guess with IPv6, NAT problem could be resolved, if you assign each devices a static public IP, then you can make direct IP dialing, that is free! Maybe only a Name server is needed for phone_number with your IP.

actualy, google talk, IM, MSN messanger all are free for internet talking, why not VOIP?

I know the protocals are different.
