把名字什麽的一填，找人witness你簽字，就好了。

又，如果孩子是美國公民的話，除非你指定或你在中國的親屬申請，政府不會把孩子送回中國的。 （當然，住forster home應該還不如讓國內的親戚朋友帶。）
