1. 個人申請， company contact information 是不是不用填寫

2. 有2個選項感覺一樣

-This one is correct for your case. Form I-140 means "Immigrant Petition for Alien Worker". EB-1A I-140 is self-petition, which mean you file I-140 for yourself. In other words, you are the petitioner and you are also the beneficial of your own I-140.

3. Part 2. Information about Request 下麵這些除了第3個，其他的4個都不知道哪裏能有
(1). Form number of related petition/application
-It is "I-140".
(2). Receipt Number of Related petition/application
-Leave it blank.
(3). Classification/Eligibility Requested: EB1a?
-Right.
(4). Petitioner/Applicant in the Relating case
-Your name
(5) Beneficiary in the Relating case
-Your name.
5. Part 4 Original Signature of Attorney or Accredited Representative 這個部分，原來以為沒有律師代理就不用填，但是後來發現下麵有這麽一行字
-Leave it blank.
Same individual as signing above in Part 3.
-You misunderstand it. There are some words "(Note if attorney is signing above in Part 3)" after "Part 4 Original Signature of Attorney or Accredited Representative". Do you know what that mean? If no, you need to improve your English.
