I907表中的一堆問題，請指點

I-907表問題：

1.       個人申請， company contact information 是不是不用填寫

2.       有2個選項感覺一樣

(1). You are the petitioner who is filing or has filed a petition eligible for Premium Processing.

     (2). You are the applicant who is filing or has filed an application eligible for Premium Processing.

3. Part 2. Information about Request 下麵這些除了第3個，其他的4個都不知道哪裏能有

  (1). Form number of related petition/application

  (2). Receipt Number of Related petition/application

  (3). Classification/Eligibility Requested: EB1a?

  (4). Petitioner/Applicant in the Relating case

  (5) Beneficiary in the Relating case

5. Part 4 Original Signature of Attorney or Accredited Representative 這個部分，原來以為沒有律師代理就不用填，但是後來發現下麵有這麽一行字

        Same individual as signing above in Part 3. 
這麽說還是要填寫了？
