company and ask for advice. It is possible the doctors bill wrong. They will refund if a mistake is made.
2) From my own experience, I do feel it is possible that some doctors got some kind of benefits by ordering expensive tests. However, without solid proof, you can not do anything about it. (They always have a "reason" to order it. Though, some tests are not as necessary. On the other hand, some people complain when those expensive tests not ordered :-()
3) You do not have to do whatever doctor asked you to do. For example, he referred an out of network doctor. You can find another doctor of same specialty inside network. You do not have to do the test the doctor ordered, either.
4) Waiting for 30min and get a 5min talk time with the doctor seems to be the norm here. Hate it! To make the waiting worth, you can prepare tons of questions and asks the doctor ;-)
1) If you think they bill extras, you should call your insurance
